Dear Patient:
We hope you and yours have been healthy and safe during these unprecedented times. Our doctors and staff have been carefully planning the safest procedures for your visit to our office. Rest assured – we remain committed to the highest level of patient satisfaction while ensuring the safest environment for our patients and staff.
We will be taking the following precautions as we welcome you back to our clinic:
Phone and in-person screening
· Patients will be screened for COVID-19 symptoms both during confirmation calls, as well as upon arrival at the office.
· All Patients and their accompanying members, as well as doctors and staff will be screened for COVID-19 symptoms, temperature and oxygen level upon entering the office.
· If you exhibit any symptoms, or believe you have been in contact with someone who may have been exposed, we ask you to reschedule your exam.
Limit number of people in the office
· We ask that you not bring a companion to your appointment, unless you need a caregiver or guardian to assist you. Accompanying adults are asked to wait in their cars.
· Parents – we LOVE your children – but please arrange for childcare at home if your child does not have a scheduled appointment. We also request that only one parent accompany each child to his or her appointment. We must try our best to adhere to CDC social distancing guidelines.
· We have rearranged our waiting areas to allow for 6 feet of physical spacing.
Bring your own mask and wear at all times in the office
· For the safety of our staff, doctors and patients – we will not allow anyone to enter the office without a mask. This includes both patients and accompanying members. If you do not have a mask, we can provide you with one.
Patient In-Take Forms
· New Patient forms will be mailed to patients before their appointment and are available for download from our website. These should be completed prior to arrival for the appointment.
Sanitizing of Office and Equipment
· All equipment and surfaces will be sanitized by our staff before each patient.
· Hand soap and hand sanitizer are available at multiple stations for staff and patient use.
· After trying on frames, our staff will sanitize the worn frames before returning to boards for display.
Please call ahead to pickup/repair eyeglasses or new contact lenses
· In order to limit the number of people inside the office at one time, we will deliver to your vehicle whenever possible. Walk-Ins are discouraged and may result in you having to wait outside until we can accommodate you.
· You may be asked to wait in your vehicle during eyewear repairs.
The world has changed but one thing remains constant: our absolute commitment to your health and well-being. We look forward to seeing you soon!
